For gaps that cannot be addressed through forward-looking mechanisms, create honest retroactive documentation. Write risk assessments clearly dated as of the current date. Document known limitations of historical governance — do not fabricate evidence. Auditors understand that governance programs have start dates. Honesty about historical gaps combined with demonstrable current governance is far more credible than fabricated documentation.
